Abstract

Cognitive Behavioral Therapy (CBT) and Relapse Prevention (RP) have demonstrated effectiveness in the treatment of Substance Use Disorders (SUDs); however, the role of coping as a therapeutic mechanism requires further integration. The aim of this systematic review (SR) was to analyze the role of coping within CBT-based RP interventions for individuals with SUDs, examining both the types of coping strategies targeted and their relationship with clinical outcomes. The SR was conducted following PRISMA guidelines and registered in PROSPERO. Randomized controlled trials were identified through searches in Scopus, Web of Science, PubMed, PsycINFO, SciELO, and Redalyc. Risk of bias was assessed using the JBI Critical Appraisal Checklist. Of the 618 initial records identified, 15 studies met the methodological eligibility criteria. Findings indicate that the interventions were effective in reducing substance use and extending periods of abstinence. Active coping, problem-focused coping, and cognitive restructuring were associated with improved clinical outcomes, particularly through their interaction with self-efficacy enhancement and reductions in anxiety sensitivity. Personalized coping skills training optimizes skill acquisition, decreases avoidance behaviors, and promotes treatment adherence and long-term sustainability, particularly among populations with comorbid conditions. It is concluded that coping should be considered a central therapeutic target within treatment programs, as strengthening coping abilities may optimize clinical outcomes and contribute to the development of more personalized interventions
